ANNEX IX
Information to be submitted with the registration of high-risk AI systems listed in Annex III, regarding testing under real conditions in accordance with Article 60
Regarding tests under real conditions to be recorded in accordance with Article 60, the following information shall be provided and kept up-to-date thereafter:

A Union-wide unique identification number of real-world testing; |

2. |
ee name and contact information of the offeror or potential offeror and the persons responsible for use involved in real-world testing; |
|
3. |
A brief description of the AI system, its intended purpose, and other information necessary to identify the system; |
|
4. |
A summary of the main features of the plan for testing under real-world conditions; |
|
5. |
information about suspending or terminating testing under real-world conditions. |
