APPENDIX V
EU DECLARATION OF CONFORMITY
The EU declaration of conformity referred to in Article 28 shall include all of the following information:
name and type of the product with digital elements and any additional information that enables its unique identification
name and address of the manufacturer or the manufacturer's authorized representative
a statement that the EU declaration of conformity is issued under the sole responsibility of the supplier
subject of the declaration (identification of the product with digital elements that can be used to trace the product; if appropriate, a photo may be attached)
a declaration that the object of the declaration described above complies with the relevant Union harmonization legislation
references to all relevant harmonized standards or other common specifications or cybersecurity certification used in relation to which the declaration of conformity is indicated
where applicable, the name and number of the notified body, a description of the conformity assessment procedure carried out, and the identification of the certificate issued;
